GREATER NASHUA MEN’S EVANGELICAL SOFTBALL LEAGUE

I, the parent or legal guardian of ________________________________________ who is
under the age of eighteen (18) years, but not less than sixteen (16) years, do
hereby give permission for him to participate in GNMESL softball games during
the __________ season.
The GNMESL, league officials, churches and coaches will not in any way be held responsible
in the event of an injury sustained while traveling to or from any contest or any
participation in games or practices.
I agree to release the GNMESL, officials, churches and coaches from any claim by said minor.
